The New Jersey Department of Health says that anyone coming into New Jersey from 22 states on the COVID-19 travel advisory will be asked to fill out a questionnaire when entering the state.
The questionnaire will be available on smart phones. The travelers are also being urged to quarantine for 14 days before going out in public.
“The self-quarantine is voluntary, but compliance is expected. It is relying on personal accountability,” said Health Commissioner Judith Persichilli.
The effort to gather information and to quarantine travelers will begin on Monday. The 22 states on the advisory list are states where COVID-19 cases are surging.
“It is vital that individuals traveling from heavily impacted states cooperate so we can avoid creating community outbreaks in our state,” Persichilli said.
The voluntary electronic survey can be accessed through any smartphone. Airlines have agreed with the state to announce the survey on departure and on flight to New Jersey.
“Travelers will be connected to an electronic survey that will collect information about where they are traveling from, their residence and their destination. The survey has the list of the travel advisory,” the commissioner said.

Travelers can access the survey through a QR code or text. The information will then be transmitted directly to county health departments.
“Once the Health Department receives the traveler’s information, we are asking them to call the travelers to remind them of their request to self-quarantine,” Persichilli said. “Individuals should leave the place of quarantine only to seek medical care or treatment or to obtain food and other essential items.”
