St. Vincent's kicks off personalized cardiology program

St. Vincent's Medical Center is helping patients with heart issues through more personalized care.
The hospital began the second phase of the American Cardiology Patient Navigator Program.
It focuses on helping patients avoid return trips to the hospital following a heart attack.
Patients are assessed for their risk of being re-admitted, given information about their recovery and are checked in on after being discharged.
St. Vincent's is one of only 15 hospitals in the country selected to be part of this program.
